Whether you’re bringing in a child for their first dental visit, maintaining a healthy smile as an adult, or managing ongoing gum health, there’s a plan built just for you. Each option includes regular preventive care, special member discounts, and the same great service you’ve come to expect. All you pay is a monthly fee.

Gusfa Dental Care Smiles For Miles Options
- Professional dental cleanings (2 per year)
- Doctor exams (1-2 per year)
- Fluoride Treatment (1 per year)
- Needed x-rays*
- One emergency exam per year (if needed)
- 15% Off all other treatment (some exclusions may apply)
- $149 Lifetime Activation Fee includes first month
For patients with regular cleanings and absence of periodontal disease. (13 and Older)
- Perio maintenance cleanings (4 per year)
- Doctor exams (1-2 per year)
- Fluoride Treatment (1 per year)
- Needed x-rays*
- One emergency exam per year (if needed)
- 15% Off all other treatment (some exclusions may apply)
- $149 Lifetime Activation Fee includes first month
For patients enrolled in active periodontal maintenance due to prior treatment of periodontal (gum) disease. (Periodontal Disease)
- Professional dental cleanings (2 per year)
- Doctor exams (1-2 per year)
